Plate 4 Life Dietetics provides personalized nutrition plans and support to help you achieve your health goals!
Initial appointments include thorough assessments and may take up to two hours. Follow-up visits generally last one hour but can vary based on the topics you wish to cover.
Plate 4 Life Dietetics is currently only self-pay at this time, but in the process of becoming in-network with insurance plans.
Payment is expected upon completion of appointment via Zelle, Venmo, or credit card. Credit card information is not requested or kept on file. An invoice is sent to client upon completion of appointment, and payment is expected upon receipt of invoice. Credit card information is provided only through merchant services provider.
A superbill is provided upon request if you wish to submit to your insurance company.
Allow up to 2 hours for your initial visit. Follow-up visits are typically 1 hour, but can fluctuate in time.
$200 per hour.
Discounts for self-pay and for individuals submitting superbill to insurance company themselves.
Self-Pay - Equitable Pricing
Billing is in 15-minute increments of $25. A one-hour visit would be $100 with a prompt payment.
Invoices are provided upon completion of appointment. Payments are due upon receipt of invoice and request for payment.
A 3% service charge is added on to total amount for credit card payments.
If you are in need of nutrition services due to a significant hardship, additional discounts are available. The amount of discount is assessed on a case-by-case basis and can be as low as $80 per hour.
