Plate 4 Life Dietetics provides personalized nutrition plans and support to help you achieve your health goals!
Initial appointments include thorough assessments and may take up to two hours. Follow-up visits generally last one hour but can vary based on the topics you wish to cover.
Plate 4 Life Dietetics is currently only self-pay at this time and does not file insurance claim on behalf of patient.
Rate is $20 per 15-minute unit, or $80 per hour, prorated based on the 7/8 rule for each 15-minute unit.
Allow up to 2 hours for your initial visit with a maximum invoice time of 2 hours for appointment time invoicing.
Follow-up visits are typically 1 hour, but can fluctuate in time.
Invoice and request for payment is provided upon dietitian sharing visit notes and any handout following client's visit.
Payments are accepted via Zelle or credit card (via merchant services). Credit card information is not requested or kept on file by dietitian. Any credit card information is shared strictly through merchant services and is not seen or shared with dietitian or Plate 4 Life Dietetics.
A superbill is provided upon receipt of payment for client to submit to your insurance company for reimbursement. It is the responsibility of the client to contact their insurance company to confirm coverage of dietetic services.
